Dear Reader,

This month the Bulletin joins over 230 other journals around the world in p=
ublishing papers on poverty and human development, as part of an initiative=
by the Council of Science Editors.

The first article on poverty and human development, an editorial, by August=
in Kwasi Fosu, examines the different measures of development and poverty <=
http://www.who.int/bulletin/volumes/85/10/07-045955.pdf> , and how these in=
dicators are linked to health. The second, a research paper by Alexander Wa=
rren Hertel-Fernandez et al., reports that infants born to the socioeconomi=
cally disadvantaged in Chile had a significantly higher risk for mortality =
<http://www.who.int/bulletin/volumes/85/10/06-041848.pdf> by infectious di=
seases and trauma during the first month of life.

In the third, a review paper, Lale Say & Rosalind Raine find that inequalit=
ies in the use of maternal care in developing countries vary widely, highli=
ghting the need to investigate and assess context-specific causes of use of=
 maternal health care <http://www.who.int/bulletin/volumes/85/10/06-035659.=
pdf> . The final paper in this set, by Tanja A J Houweling et al, found tha=
t poor-rich inequalities in maternity care <http://www.who.int/bulletin/vol=
umes/85/10/06-038588.pdf> in general, and professional delivery care in pa=
rticular, are much larger than those in immunization coverage or treatment =
for childhood illnesses.

Seven papers in this issue of the Bulletin focus on maternal health. In an =
editorial introducing these papers, Monir Islam describes the enormous chal=
lenges to reduce child and maternal mortality <http://www.who.int/bulletin/=
volumes/85/10/07-045963.pdf> in the develop=ADing world faced by the Safe =
Mother=ADhood Initiative in the 20 years since it was formed.

Guo Sufang examines the increase in Caesarean section <http://www.who.int/b=
ulletin/volumes/85/10/06-035808.pdf> in China. Nils Chaillet et al. examin=
e the attitude of obstetricians in Quebec to clinical practice guidelines <=
http://www.who.int/bulletin/volumes/85/10/06-039289.pdf> targeting the man=
agement of labour and vaginal birth after a previous Caesarean birth.

Community-based distribution of injectable contraceptives <http://www.who.i=
nt/bulletin/volumes/85/10/07-040162.pdf> is common=ADplace in some countri=
es in Asia and Latin America, but is practically unknown in Africa. A commu=
nity trial in Uganda, by John Stanback et al., shows that well trained comm=
unity health workers can safely provide contraceptive injections.

Steven A Harvey et al. found that skilled birth attendants <http://www.who.=
int/bulletin/volumes/85/10/06-038455.pdf> in Nicaragua lack the knowledge =
and skills to manage common life-threatening obstetric complications.

Use of magnesium sulfate for pre-eclampsia and eclampsia is below desired l=
evels in Mexico and Thai=ADland, according to a study by Pisake Lumbiganon =
et al. Laurel Hatt et al. conclude that the poorest women still find it har=
d to access emergency obstetric care <http://www.who.int/bulletin/volumes/8=
5/10/06-037911.pdf> .

In an editorial, Sara Wood discusses how policy-makers and scientists often=
 lack access to data <http://www.who.int/bulletin/volumes/85/10/07-046649.p=
df> they need to assess priorities for public health. She explains how inf=
ormation that can prevent disease outbreaks moves slowly from wealthy to de=
veloping countries. In the news, Jos=E9 Orozco reports on the spread of de=
ngue fever <http://www.who.int/bulletin/volumes/85/10/07-011007/en/index.ht=
ml> in Venezuela. Claire Keeton reports from South Africa on the challenge=
s of doing clinical trials involving children <http://www.who.int/bulletin/=
volumes/85/10/07-021007/en/index.html> . Cininta Analen reports from Indone=
sia on a novel ap=ADproach to improving maternal health care <http://www.wh=
o.int/bulletin/volumes/85/10/07-031007/en/index.html> in rural areas. In t=
his month's interview, Michael Kazatchkine, Executive Director of the Globa=
l Fund to fight AIDS, Tuberculosis and Malaria, talks about plans to start =
financing health systems <http://www.who.int/bulletin/volumes/85/10/07-1010=
07/en/index.html> in developing countries.

Ryuichi Komatsu et al. assess the contribution of Global Fund-supported pro=
grammes <http://www.who.int/bulletin/volumes/85/10/06-038315.pdf> towards =
achieving international targets and Millennium Development Goals in terms o=
f fighting HIV/AIDS, tubercu=ADlosis and malaria. They conclude that consid=
erably greater financial support will be needed to meet 2009 targets.?
