Dear Reader,
This special theme issue <http://www.who.int/bulletin/volumes/85/5/en/index=
.html> of the Bulletin is timed to coincide with a resolution on tuberculo=
sis (TB) control to be considered the 193 Member States of the World Health=
Organization at the World Health Assembly from 14 to 23 May.
L=E9opold Blanc & Lindsay Martinez introduce this theme issue on TB control=

l on the importance of the two key targets <http://www.who.int/bulletin/vol=

ground-breaking 1966 paper by Halfdan Mahler, WHO Director-General from 197=
3 to 1988, on tuberculosis programmes in developing countries that was publ=
ished in the Bulletin of the International Union Against Tuberculosis and L=
ung Disease.
The WHO-recommended TB control strategy includes supervised administration =
of treatment under proper case management conditions, or directly observed =
treatment. Critics advocate eliminating supervised administration <http://w=
ww.who.int/bulletin/volumes/85/5/06-038927.pdf> from TB treatment guidelin=
es, substituting self-administration or family treatment observation. But T=
homas R Frieden & John A Sbarbaro argue that this advocacy is based on misi=
nterpretation of scientific data and failure to rigorously analyse public h=
ealth practice.
Paul Garner et al. cite the failure of TB patients to complete their treatm=
ent <http://www.who.int/bulletin/volumes/85/5/06-035568.pdf> , even in plac=
es where health services are good, as a major obstacle for TB control. They=
call for new approaches to overcome the stigma of TB and to tackle gender =
differentials in TB treatment.
