I'm pretty sure it's not that.  I am on PACER because I receive
electronic notifications of cases in which I am counsel of record. 
However, many of the notices are nothing more than clerk's entry that
a paper has been filed.  Unless a document was filed electronically,
all I get is a line describing the nature of the document.  (Documents
that the court itself generates are filed electronically.)  However, I
must pay per page for documents filed in cases in which I am not
counsel of record.
