I already rolled my own and faxed it a few days ago.

Anyone who'd like to see it, e-mail me.  Be warned, it is NOT in
Microsoft Word format, but is a Describe word document (they
were a competitor to MS Word).  Otherwise I could go way out
on a limb and make the exception of this new year and figure
out how to turn it into a plain text document!  (But that might
pose a hardship all round.)
