Look on page 17411 of the April 19, 1996 Federal Register. This is
the publication of proposed Maximum Achievable Control Technology
regulations for hazardous waste combustors.
In this table, applicable current and proposed regs are shown for
large municipal waste combustors, proposed medical waste incinerators,
European Union hazardous waste combustors, and proposed regs
for hazardous waste burning incinerators, cement kilns and lightweight
aggregate kilns. Cement kilns and lightweight aggregate kilns burn
about 2/3 of all commercial hazardous waste.
The nice thing about the table is that all of the standards are converted to
common units for comparison.

Chlorinated dibenzo dioxin/furan is measured by one shot stack tests.
They will typically do 3 runs of 3-4 hours each and do analytical work to
arrive a dioxin results for each of the three runs, which are then averaged
for compliance purposes.
As of now, I am unaware of any continuous emission monitoring technology
that measures PCDD/PCDF. During compliance demonstrations at
RCRA regulated facilities, "surrogate" measures of combustion temperature
and residence time, flue gas temperature at the air pollution control system
inlet, ESP power consumption or baghouse pressure drop, feed rate restrictions,
destruction and removal efficiency and other operating parameters are
determined and written into permits as "dioxin-control" provisions.
I have heard of some PCB continuous technology, but I don't know anything
about it right now...except that I heard it was very expensive.
EPA's proposed hazardous waste combustor rules (in the April 19, 1996 FR)
lays out the case for the commercial availability of continuous emission
monitors for toxic metals based on European technology.
