chlorine accidents

To all recepients of dioxin discussion list:

I am working on a PhD project on the (innovation) strategies of PVC-
linked companies pursue in response to the chlorine and PVC debates.
Special focus is on the question how and why companies choose between
optimizing the PVC production chain and switching over to substitutes.

Right now, I am looking for case-histories of chlorine accidents
(and/or VCM and other PVC related accidents and issues). In that
sense the dioxin list subscribers already provided some useful
information! However, I was wondering if anyone knows whether
there exist monitoring lists of such accidents? Or do you have
information on specific accidents involving chlorine or PVC related
substances?
I would be very grateful for any information on this!

Does anyone know about WEB-discussion lists or sites more
specifically focused on chlorine and/or PVC?

For other people interested, I already found the following sources
on accidents:
- V.C. Marshall (1987) 'Major Chemical Hazards'
- Manufacturing Chemist's Association, Inc (1962, 1966) "Histories of
accidents in the chemical industry"
- Advisory Committee on Major Hazards (1979, 1984) "Second Report",
"Third Report" HMSO
